Amy is a life-long learner, problem solver and catalyst for improving people’s lives. She always wanted to play a role in helping people navigate the healthcare system. This opportunity came-to-be when she joined a two-year diabetes and hypertension grant program through the Centers for Disease Control and Prevention and the State of Connecticut Department of Health. “I performed MTM (medication therapy management) services for 40 patients four times per year. We helped them set and achieve goals for improving their diabetes or hypertension.” The Pequot Pharmaceutical Network (PRxN), where she serves as Clinical Pharmacy Manager, was selected for the grant due to the population it serves, the Mashantucket Pequot Tribal Nation, which has a higher than usual risk for heart disease and diabetes. Although the team dealt with challenges such as scheduling, patient involvement and medication adherence, “I was able to develop clinical pharmacy services and protocols to ensure the provision of safe, effective and economical pharmacotherapy for our patients.”
